Important Upcoming Health-Related Dates at TAPA!
All of the following screenings are done annually and are complete for the School Year 2018/19!
Flu Clinic for students and staff: Wednesday, 10/10/18 from 8:30am-10:00am. DONE!
HPV Clinic for Lower School Students: Tuesday, 11/13/18 from 10:00am-11:30am. DONE!
Senior Meningitis Vaccine Clinic: Wednesday, 11/28/18 from 10:30-11:30. DONE!
Scoliosis Screening for 7th (and new 8th) graders on Mon., 3/18/19. All 7th graders should wear a button-down shirt that day that they can wear backwards during screen. (for modesty). DONE!
Dental Screening for 7th (and new 8th) graders on Wed., 4/24/19 at 9am. If a parent would like to opt out of this screening, simply send in that request in writing prior to that day. Thanks! DONE!
Vision Screening (thanks to the Lions Club Volunteers) for 7th and 9th graders on Thursday, 5/2/19 at 9:30am. DONE!
VACCINES
We have worked hard to get our vaccination rate to 100% compliance!!!!!!!!! Next year, this work will continue so these rates will follow! REMEMBER ~ If your child is exempt to a certain vaccination for religious or medical reasons, this Exemption Form is due annually, in August! Here are the links to these forms if you need them.

IMPORTANT: Communicate Your Child's Health Condition
PARENTS/GUARDIANS ~ please be sure to call the School Nurse, Ms. Tuck, to discuss any health condition(s) your child may have.
Good, and timely, communication is key to keeping your child healthy and safe at school! Nurse will write an IHP (Individualized Healthcare Plan) when necessary so that all teachers are aware of the condition and what to do in case there is an issue here at school. So important! Please keep in touch!

* Special vaccines for you! Pay attention to the VBYG (Vaccinate Before You Graduate) date here at TAPA and sign up! You definitely want to get your Meningitis booster before going to college!!!
* You may come to the nurse anytime in the Spring and ask for a copy of your vaccines for your records. You may need a copy for a future job and/or the college you attend. Try to do it BEFORE the last day of school!

Forms, Forms, Everywhere A Form!!!

We know! They can be overwhelming! BUT, they are required. Below is helpful information so you'll know which forms your child needs. See also the appropriate forms which are attached - please print off, complete, get doctor's signatures when necessary, and turn those forms into Ms. Tuck ASAP.
FOR EVERY STUDENT:
* STATE PHYSICAL (due at the start of 7th Grade)
- * HEALTH HISTORY (due at the start of every school year, EVERY student 7-12)
- * OVER-THE-COUNTER PERMISSION (due at the start of every school year, for EVERY student 7-12)
For students who take prescription meds at school:
- DISPENSING MEDICATION AT SCHOOL ( only if your child needs to take a RX medication during school hours) -
For students with Asthma only:
- INHALER AT SCHOOL form (to carry inhaler in backpack, HIGHLY encouraged to do)
- ASTHMA ACTION PLAN
- and they'll also need a DISPENSING MEDICATION AT SCHOOL if they keep a spare inhaler at school (again, HIGHLY recommended)
For students with Serious Food Allergies only:
- FOOD ALLERGY ACTION PLAN
For students with Diabetes only:
- PHYSICIAN ORDERS FOR STUDENTS WITH DIABETES
- and they'll also need a DISPENSING MEDICATION AT SCHOOL if they keep spare supplies (glucometer, Insulin, Glucagon, etc) at school with Nurse (again, HIGHLY recommended)
For Students with Seizure Disorders only:
- SEIZURE ACTION PLAN
- and they'll also need a DISPENSING MEDICATION AT SCHOOL if keeping a medication for it here at school
