    SCHEDULE CHANGE REQUEST FORM

     TAPA Stars - this form is to submit any academic issues you are having with your schedule.  For example, if you were placed in regular level classes but would like to challenge yourself in Honors, or if you accidentally got placed in the wrong type of Spanish class, this is the form.  This form is not for "I want to change my advisory and all my classes!" however, if you have any questions or concerns you are welcome to email Ms. Bryant.  It is important to us that your voice is always heard so that you can learn!
